Letter templates for employment matters
The generators create a ready PDF or DOCX file from your answers. Your data stays in your browser. We do not send it to a server and we do not submit letters on your behalf.
The document is created in Polish. The letter goes to a Polish authority or employer, and Polish is the official language in Poland (Article 27 of the Constitution). The form and tips are translated, but we generate the letter in Polish. Write your description of the matter in Polish or ask someone to help you translate it.
Complaint to the National Labour Inspectorate
A letter to the District Labour Inspectorate with the employer's details, a description of the breaches and a confidentiality statement.
Demand for payment of outstanding wages
A letter to the employer with a list of outstanding amounts, a payment deadline and a claim for statutory interest for late payment.
Termination of an employment contract by the employee
The generator calculates the notice period and the last day of the contract. Also termination without notice due to the employer's fault.
Termination of the contract by mutual agreement
An employee's request or a ready-to-sign agreement, with clauses on who initiated it and on leave.
Termination of a contract of mandate
A letter to the client with immediate effect or with the notice period from the contract, and a demand for settlement.
Leave request and leave on demand
A request for annual leave, leave on demand or unpaid leave: dates, number of days, cover.
Request for an employment certificate or its correction
When the employer has not issued the certificate, issued it with errors, or you need a copy.
Claim to the labour court for payment of wages
A claim with the value of the dispute, interest, grounds and a list of evidence. The case needs assessment.
Demand for payment under a mandate, specific-work or B2B contract
A pre-court demand to the client or business partner: amounts, deadline, interest, notice of a claim.
Request for the contract and settlements from a fleet partner
For couriers and app drivers: a copy of the contract, a statement of hours, a breakdown of deductions, information about ZUS.
Reporting an accident at work
Notification to the employer or client of an accident, with a request for an accident report or accident card.
