Request for the contract and settlements from a fleet partner: generator
You have the right to know what contract you work under, how many hours were counted and what money was deducted for. This letter organises the demands and sets a deadline for a reply. It does not name any intermediary company; you enter the details yourself.
Only your browser processes the data you enter in the form. We do not save it or send it to a server. It disappears when you close the tab.
The document is created in Polish. The letter goes to a Polish authority or employer, and Polish is the official language in Poland (Article 27 of the Constitution). The form and tips are translated, but we generate the letter in Polish. Write your description of the matter in Polish or ask someone to help you translate it.
How to use the generator
- The letter does not accuse anyone or judge the company. It lists the documents you are entitled to receive and sets a deadline.
- You can check your ZUS registration yourself in your PUE ZUS / eZUS account.
