Reporting an accident at work and requesting a report: generator
Report the accident immediately, preferably also in writing, so there is a record of the date and circumstances. The employer must appoint an accident investigation team and draw up a report within 14 days; under a mandate, the client draws up an accident card.
Only your browser processes the data you enter in the form. We do not save it or send it to a server. It disappears when you close the tab.
The document is created in Polish. The letter goes to a Polish authority or employer, and Polish is the official language in Poland (Article 27 of the Constitution). The form and tips are translated, but we generate the letter in Polish. Write your description of the matter in Polish or ask someone to help you translate it.
How to use the generator
- The employee must notify their supervisor of the accident immediately (Article 211(6) of the Labour Code). A written report is not required, but it protects you from a dispute over whether and when the report was made.
