The document is created in Polish. The letter goes to a Polish authority or employer, and Polish is the official language in Poland (Article 27 of the Constitution). The form and tips are translated, but we generate the letter in Polish. Write your description of the matter in Polish or ask someone to help you translate it.

How to use the generator
- You send the complaint to a District Labour Inspectorate. According to the PIP, the competent one is determined by the employer's registered office. Each OIP covers one voivodeship (M.P. 2007 item 1006).
- Describe the facts: what, when, who is affected. You do not need to cite legal provisions.
- If you work under a civil-law or B2B contract and want the PIP to check whether it is actually an employment relationship, tick this in step 3. How the PIP establishes an employment relationship.
